The main cause has been the concealment garment or part of the holster getting into the trigger guard.

The first thing to look at is the draw string on a jacket. Secondly is worn leather holsters, if the leather becomes pliable it can fold into the trigger guard.

The last concern is the garment falling in the holster and then getting into the trigger guard.

In conclusion, maintain the condition of your holster, tie up the loose ends on your jacket draw strings and perform a garment sweep after reholstering.
